Initial Health consult
$350 - 90 minute consult
- Review medical history and health goals

Follow up consults

- 2 (60 min) consults for per family member
- 6 (30-min) follow-up visits per member
- Unlimited messaging M-F clinic hours
- 10% discount on supplements
- Additional visits 20% off normal rates
- No lab draw fees
- Lab Fees not included
- No prescription refill fees
- In person, phone, telemedicine available
- Cancellation of plan prior to 6 months will incur a graduated fee to cover the services to date. (See contract for details)
- Option to pre-pay for the entire year: $2400 annually (Savings of $300)

$375 / Month for 2 family members
$150 / for each addition family member
